You most certainly can OD on ketamine and die.
It's actually pretty hard, unless you're like Matthew Perry and you've got a doctor administering it to you in the bathtub (while on a cocktail of other substances). You're going to become incapacitated long before you can administer enough to yourself to end your life... It will fuck up your bladder if you abuse it long term though (and apparently he's having these issues).
The LD50 for ketamine is very high, which is why it's used as anesthesia for toddlers.
The real move would be for his provider to just slip a tiny amount of fentanyl in there...
