Japan Airlines plane strikes parked Delta jet at Seattle-Tacoma International Airport

When this happens I always wonder how much does someone (or multiple someones) have to screw up?

Surely (don't call me Shirley) the patterns are designed with large buffers to allow for some error?

(I don't know anything about airport design, but this concept is used in other spaces, EG road intersections).
