This could be as discreet as seeing someone changing behind their car (that's the only time it has happened to me), or as blatant as walking up on people having sex on a hiking trail (something that happened to my ex gf).
I went hiking at the top of a hill above an old abandoned quarry, and spotted some people down below swimming in the quarry lake, two girls and two guys. Both of the guys were naked and one of the girls was. I watched from a distance and hidden under the shade of a tree for about half an hour, then wandered off.

For the most part, it's like you said: someone changing clothes, or losing them via unintended consequences like jumping into a pool wrong and not realizing it.
Many years ago, my family went camping to one of our favorite spots. As usual, I set up my tent close to the creek. I have a few occasions.
At some point in life, I lived in a newly developed area and I liked to take some long walks to clear my thoughts and... do stuff by myself, on the hills.
