Further info. OP probably knows it, I'm sharing it for the others.
The two kids there were most likely Geta (bottom left) and Caracalla (bottom right). Caracalla ordered Geta's death, and condemned him to damnatio memoriae (damnation of memory; basically "Geta is now an unperson"). Geta's face was likely scrapped off the portrait due to that damnatio memoriae, and there are signs human faeces were smeared on its place.
Also, the portrait was initially square or rectangular, but some muppet (potentially in modern times) cut it round; note how the tips of the sceptres are missing.
